Nothing said, done, performed, typed, printed or produced by Carrie Cummings, Ray of Light Mentoring, its officers, representatives, or associates are meant to diagnose, prescribe, treat a disease or take the place of diagnosis by a licensed physician, therapist, psychologist or psychiatrist.
I understand that sessions and techniques used by Carrie Cummings are intended only to promote healthy lifestyle and cultivate the mind/body/spirit connection.
I understand that in a session, a variety of non-invasive methods, modalities, and programs may be used, including, but not limited to, IPT, guided imagery, applied kinesiology (muscle response testing), NLP, chakra work, feelings, role play, emotional release work, guided visualization, breath work, inner child work, rebirth, meditation, life coaching and creative healing work. I understand that it is the purpose of the facilitator in using these techniques to bring the client to a place of inner healing, and to create a strong sense of well-being and closure with the issues being addressed.
I understand and acknowledge that I am granting my permission for Carrie Cummings to process my family member, a person for whom I am a legal guardian, or myself. This authorization is valid for any and all sessions with Carrie Cummings.
I also understand and agree to the terms of Carrie Cummings’ “no show and cancellation policy”. I understand that Ray of Light Mentoring requires a 24 hour cancellation or I will be charged $50. I can always reschedule, but will agree to pay the $50 if the cancellation is less than 24 hours notice. If I do not show up for the appointment without any notification I will be charged for the entire cost of the session.
I hereby assume full responsibility for arranging these emotional processing sessions and release and discharge Carrie Cummings from any and all claims, liabilities, damages, actions, or causes of action arising from the services received hereunder.
